Are You Unhappy About The Services That You're Getting?

If you are in a nursing facility, you may contact the Ombudsman and explain your concerns.

  • If you are under the age of 60, have a physical disability, and are receiving services in your home or community first tell your provider about your concerns or complaints. If you are not satisfied with the provider’s response, contact your supports coordinator.
  • If you are over the age of 60 and are receiving services in your home or community first tell your provider about your concerns or complaints. If you are not satisfied with the provider's response, contact your care manager.
  • If you are receiving federal or state funded home and community based services, contact the Office of Long Term Living toll free number at 1-800-757-5042
