Discharge From The Hospital Or A Rehabilitation Center

Talk to the social worker and/or discharge planner at the hospital or center to find out what services may be needed during recuperation at home.

For those age 60 or older, call your Area Agency on Aging and ask for an assessment of needs and assistance in arranging services.

For those with disabilities, call the Center for Independent Living or Disability Enrollment Agency in your area.
